"Writer" and "Story / Teleplay" differences[edit]

There are several episodes in season 2 (by now, only) - Blood, Colony, Dod Kalm, Anasazi - that don't have any writer. Instead, they have some names in "story" and "teleplay" spaces given. Sometimes those names repeat, sometimes not. My question is, what is the difference between "story" and "teleplay" and between those two and "writer".
