Thanks for bringing it to my attention. As you can see on their talk page, the IP belongs to a school system in Indiana and they've been repeatedly blocked before for consistent vandalism. I'll keep an eye on their contributions to make sure they don't get out of hand again.

Thanks for letting them know about their vandalism on their talk page; and for some "official" warning templates, check out WP:TUSER. There's a whole list of warnings and the syntax to add them to a user's talk page to warn them. OR, if you desire, you can add the warnings directly to your toolbox on the left side under the search area. Take a look here for how to do so.
