Sorry, I only just got your email. I was using a forwarding address and it seems to have turned itself off sometime in April without my knowledge.
Did you already upload a file of the DVD cover? I couldn't find an upload. Instructions for upload are at Wikipedia:Uploading images#Procedure to upload or click on the "upload file" link in the left-hand side "tools" menu. If copyrighted, the cover art should be low resolution, have a descriptive file name (such as "File:Mumia cover art.jpg") and include a fair use rationale template such as Template:Non-free use rationale video cover.
Once uploaded, the file name only is put in the image parameter of the infobox. So, if the file was called "File:Mumia cover art.jpg" then the wikitext would look like:
image = Mumia cover art.jpg
