Where did the cover come from?

Has anyone actually confirmed the cover?? I 'm seriously convinced no artist would ever release a CD with such a terrible cover, it looks user-made. Should I find no reliable source on the cover and/or an explanation for its appearance, I will delete it within the next 48 hours.
