Q1: Why doesn't the article describe the territory as "Pakistan-occupied Kashmir"?
A1: For the same reasons that Jammu and Kashmir isn't described as "Indian-occupied Kashmir". Wikipedia follows usage in reliable sources and adheres to a neutral point of view.
Q2: How about "Pakistani-administered Kashmir"?
A2: Pakistani-administered Kashmir is normally used as a cover term for Azad Kashmir + Gilgit-Baltistan.
Q3: But is "Azad Kashmir" a neutral name?
A3: It's up to the article to describe that in detail. Regardless, an article's title should use the most common name, and that doesn't have to be neutral.
